At the end of the school year/beginning of the summer the last thing on the mind of any parent is a back to school physical. However this is the best time to get it done. Avoid the rush and stress by taking care of it now.

Perhaps the most important decision you make about the pre-school physical is where it takes place. Nowadays, there are many health care outlets that advertise and perform these evaluations quickly and cheaply. While they may be able to fill out a piece of paper and perform a basic exam, is it really what your child needs? Quite simply – no!

While it is possible to have mass sports physicals done in schools or physicals at a minute clinic, it cannot take the place of an annual checkup with your pediatrician. You always want you r child to have a comprehensive health check in his/her medical home- with a pediatrician that knows or can review their personal and family history.

Health checks are extremely important because it is at this time that the clinician assesses overall health, catches problems early, updates vaccines if necessary, reinforces healthy behavior, and reviews developmental milestones.

The following will be evaluated at the time of a back to school physical

Frequently Asked Questions

My child's school is asking for a Georgia form 3300 and 3231. What are those? How soon can I get one?
The GA-Form 3300 is also called the Eye Ear and Dental form. It is a reporting of basic in office screening. The GA-Form 3231 is a record of immunizations. Both forms can be provided on the same day as your child's visit.

My child will be a new patient to the practice. What do I need to bring to the office for my child's visit?
In addition to ID and insurance cards, you will need to provide a copy of your child's medical records which will also include the vaccine records for review.

I do not have any insurance. What will the visit cost?
Call our office at (678) 407-4489 and ask about our discounted rates for self pay patients.

How far in advance do I need to call for an appointment?
To get the ideal appointment, you want to call as far in advance in possible. However, if you missed that window, not to worry. We are able to give appointments for well checks in as little as 24 hours especially if your schedule is flexible.

I can't take any time off work; do you have any evening or Saturday hours?
Yes! We will have late appointments on Tuesday and Thursday and Saturday appointments starting the 2nd week of July just for you. You can always talk with our staff for assistance in getting appointments that will work with your schedule.

Can I have my child's school forms faxed to me?
For reasons of medical information privacy we do not fax completed forms. Forms may be picked up by the parent or legal guardian.
