While they may be able to fill out a piece of paper and perform a basic exam, is it really what your child needs? Quite simply – no!
While it is possible to have mass sports physicals done in schools or physicals at a minute clinic, it cannot take the place of an annual checkup with your pediatrician. You always want you r child to have a comprehensive health check in his/her medical home- with a pediatrician that knows or can review their personal and family history.
Health checks are extremely important because it is at this time that the clinician assesses overall health, catches problems early, updates vaccines if necessary, reinforces healthy behavior, and reviews developmental milestones.

The GA-Form 3300 is also called the Eye Ear and Dental form. It is a reporting of basic in office screening. The GA-Form 3231 is a record of immunizations. Both forms can be provided on the same day as your child's visit.
In addition to ID and insurance cards, you will need to provide a copy of your child's medical records which will also include the vaccine records for review.
